Section 28-17-13 - Maternity home, clinic and hospital reports

Each person in charge of a maternity home, clinic or hospital shall report to the department on or before the fifth day of each month a complete list of births and stillbirths that occurred in the institution during the preceding calendar month. The list shall include the child's name and date of birth, and the name of the attending physician and shall be submitted on a form provided by the department.
