In lieu of the words "(Designate office)", insert "(name of specific licensing board in the professional licensure division), Professional Licensure, Department of Public Health, Fifth Floor, Lucas State Office Building, Des Moines, Iowa 50319-0075".
In lieu of the words "(AGENCY NAME)", the heading on the petition should read:
BEFORE THE (insert the name of the specific board in the professional licensure division)
