Chapter 97 - Death Registration and Disposition of Dead Human Bodies
Rule 641-97.18 - Additional Record by Funeral Director

(1) In addition to filing any certificate or other form required by Iowa Code chapter 144, a funeral director or other person who removes from the place of death or transports or completes final disposition of a dead human body or fetus shall maintain a record which shall identify the following:
a. Name of the
deceased;
b. Date, time, and place
of death;
c. Name and address of
the person to whom the dead human body or fetus is released;
d. Name of institution or other place of
death releasing the dead human body or fetus;
e. Date of removal from the place of death;
and
f. Place and method of final
disposition of the dead human body or fetus.
(2) Records maintained under this rule shall be retained for a period of not less than ten years at the funeral establishment responsible for disposition and shall be made available for inspection by the state registrar upon demand.
