Rule 641-131.8 - Certification Denial

(1) An individual who has been denied certification by the department may appeal the denial and request a hearing on the issues related to the licensure denial by serving a notice of appeal and request for hearing upon the department not more than 20 days following the date of mailing of the notification of certification denial to the individual. The request for hearing shall specifically delineate the facts to be contested at hearing.
(2) All hearings held pursuant to this rule shall be held pursuant to the process outlined in this chapter
