Rule 481-68.6 - Expiration of Program Certification

(1) Unless conditionally issued, suspended or revoked, certification of a program shall expire at the end of the time period specified on the certificate.
(2) The department shall send recertification application materials to each program at least 120 calendar days prior to expiration of the program's certification.
