Rule 481-941.7 - Duplicate Certificate for Establishments

(1) A duplicate certificate shall be required if the current certificate is lost, stolen or destroyed. A duplicate certificate shall only be issued under such circumstances.
(2) A duplicate establishment certificate shall be issued upon receipt of a completed application and receipt of the fee as specified in 481-subrule 507.5(5).
(3) If the board receives a completed application stating that the owner of the establishment has not received the certificate within 60 days after the certificate is mailed by the board, no fee shall be required for issuing the duplicate certificate.
