Rule 4 - Registration of Licensed Physical Therapists and Physical Therapists' Assistants
Section 4-3 - Reinstatement of delinquent license

Sec. 3.

(a) A physical therapist or physical therapist assistant who is less than three (3) years delinquent in renewing a license or certificate shall be reinstated upon satisfaction of the requirements in IC 25-27-1-8(c).

(b) If more than three (3) years have elapsed since the expiration of a license or certificate, the applicant shall meet all requirements of 842 IAC 1-3-1 except that, where the applicant has not practiced for more than three (3) years, the board may, after an appearance before the board, require the applicant to retake and pass the examination provided by the board in addition to completion of continuing competency courses.
