Section 1-18 - Changes in application information

Sec. 18.

During the period for which a license has been issued, the licensee shall report to the commission changes in information provided on the license application, including the following:

(1) Current legal name.

(2) Marital status.

(3) Permanent address.

(4) Pending criminal complaints.

(5) Criminal convictions.

(6) License suspensions of ten (10) days or more.

(7) License revocations or fines of five hundred dollars ($500) or more in other jurisdictions.

(8) Racing related disciplinary charges pending in other jurisdictions.

Such information shall be upon the appropriate commission form, signed by the licensee, and filed at the commission offices within ten (10) days of change.
