Sec. 11.

(a) The school's buildings, grounds, and equipment are adequate for the age and number of students. The size and design of the building are appropriate to meet program and personnel needs.

(b) The school has a plan for the regular maintenance of buildings, grounds, and equipment.

(c) The school meets applicable rules of the fire prevention and building safety commission and applicable rules of the state board of health [sic., Indiana state department of health].

(d) Playgrounds/athletic fields are adequate in size and number and conducive to safe play.
