Sec. 5.
(a) It is the responsibility of the public utility company to obtain the necessary report forms and timely file the required reports with the department.
(b) The department will furnish each public utility company with the appropriate forms to complete their respective annual reports. However, the obligation to file the required report is not diminished or affected by the failure of the department to deliver or mail forms to the public utility company. It is the responsibility of the public utility company to obtain the necessary report forms and timely file the required reports with the department.
