Sec. 122.
(a) The center shall devise and place a daily record chart in each infant and toddler room. This chart shall provide space to record information about each child as follows:
(1) Food and fluid offered and taken.
(2) Time of diaper changes.
(3) Unusual mood of the child.
(4) Unusual health conditions, such as:
(A) nose bleeds;
(B) skin rash;
(C) elevated temperature;
(D) signs of constipation or diarrhea;
(E) injuries; and
(F) special health needs.
(b) The center shall keep charts on file for at least one (1) month.
