Sec. 44.
(a) The child caring institution shall establish and maintain an individual record for each staff member.
(b) The record shall include the following information prior to employment:

(d) If the personnel records are maintained at an office located apart from the group home residence, a copy of the health record of the direct care worker and the name, address, and telephone numbers of the person to be notified in the event of an emergency shall be kept on the premises of the group home.
