Sec. 30.
(a) To be approved to provide transportation services, an applicant shall be one (1) of the following:
(1) A community mental retardation and other developmental disabilities centers.
(2) A community mental health center.
(3) A child care center licensed pursuant to IC 12-17.2-2-4.
(4) Otherwise approved under this rule.
(b) To be approved to provide transportation services, an applicant shall certify that, if approved, transportation services will be provided using only persons having a valid Indiana:
(1) operator's license;
(2) chauffeur's license;
(3) public passenger chauffeur's license; or
(4) commercial driver's license; issued to the person by the Indiana bureau of motor vehicles to drive the type of motor vehicle for which the license was issued.
