Sec. 4.
(a) A vehicle modification evaluation must be obtained prior to the provision of any vehicle modification services.
(b) Each vehicle modification evaluation used to determine the specific vehicle modification needs of an eligible individual must:
(1) be no more than twelve (12) months
old;
(2) include any assessment of
the eligible individual and the vehicle to be modified;
(3) be provided by an evaluator of the
eligible individual's informed choice who is qualified in accordance with
section 5 of this rule;
(4) include
a recommendation for a premodification vehicle inspection as applicable in
section 6 of this rule; and
(5) be
documented in a written evaluation in the eligible individual's record of
services.
