Section 8-3 - Transfer of individual's records upon change of provider

Sec. 3.
If an individual changes providers for a home and community based service, the original provider shall transfer copies of all records related to the individual to the new provider:
(1) within five (5) calendar days; and
(2) in compliance with HIPAA regulations.
Transferred from the Division of Disability and Rehabilitative Services ( 460 IAC 1.2-8-3 ) to the Division of Aging ( 455 IAC 2-8-3 ) by P.L. 153-2011, SECTION 21, effective July 1, 2011.
