Section 20-2 - Emergency telephone numbers

Sec. 2.
(a) A provider designated in an individual's care plan as responsible for providing CMS shall recommend and advise that an emergency telephone number list should be located:
(1) in an area visible from the
telephone used by an individual; or
(2) as indicated in the individual's care
plan.
(b) The emergency telephone list shall include the following:
(1) Information given to the individual by
the individual's provider of CMS.
(2) The local emergency number.
(3) The telephone number of the individual's
legal representative or advocate, if applicable.
(4) The telephone numbers specified in the
individual's care plan, including, but not limited to, telephone numbers for
the following:
(A) The local area agency on
aging office.
(B) The provider of
CMS to the individual.
(C) APS or
CPS, as applicable.
(D) DDARS'
designated complaint number.
(E)
All other service providers identified in the individual's plan of
care.
