Sec. 15.

(a) An owner or operator of a facility where a lead-based paint activity will occur shall pay a fee of fifty dollars ($50) for each project for which a notification is required.

(b) An owner or operator who submits quarterly notifications shall be notified in writing by the department of the amount of fees due. The amount owed will be determined by the number of notifications submitted to the department in the previous quarter.

Fees shall be paid:

(1) by person or by mail;

(2) by check or money order payable to the Indiana department of health; and

(3) addressed to Cashier, Indiana Department of Health, P.O. Box 7236, Indianapolis, Indiana 46207.
