Section 2-10 - Duplicate lead-based paint program licenses

Sec. 10.

(a) To replace a lead-based paint program license that has been lost or stolen, a person shall submit a completed application for a duplicate license on a form provided by the department.

(b) The form shall include a statement indicating that the original lead-based paint program license was lost or stolen.

(c) The department shall issue no more than two (2) duplicate licenses to any person in any calendar year.

(d) The application must be submitted in person to the department by the licensee. Two (2) pieces of identification must be shown at the time of application. Acceptable pieces of identification include a valid state-issued driver's license, an Indiana issued identification card, a valid United States passport, or a valid Immigration and Naturalization Service (INS) identification, one (1) of which must include a picture of the applicant.
