Section 2-6 - Renewal of license

Sec. 6.
(a) In order to renew its birthing center license, the center shall file an application for the renewal of a birthing center license with the division at least one (1) month before the expiration of the current license.
(b) The renewal application includes the following:
(1) An application for the renewal
of a license to operate a birthing center on a form prescribed by the
division.
(2) Documents required by
the application for the renewal of a license to operate a birthing
center.
(3) The appropriate license
fee.
(c) Upon determination by the commissioner that the applicant has met the licensing requirements for a birthing center, the commissioner will approve the application for the renewal of a license to operate a birthing center and issue a license.
