Rule 3 - License Requirements for Abortion Clinics That Perform Drug Induced Abortions

Sec. 6.

(a) In order to renew its abortion clinic license, the clinic shall file an application for the renewal of an abortion clinic license with the division at least one (1) month before the expiration of the current license.

(b) The renewal application includes the following:

(1) An application for the renewal of a license to operate an abortion clinic on a form prescribed by the division to include the selection of only one (1) of the following procedure classifications:
(A) Drug induced abortions only. The clinic is precluded from performing surgical abortions.

(B) Both drug induced abortions and surgical abortions. The clinic must comply with this article and 410 IAC 26.

(2) Documents required by the application for the renewal of a license to operate an abortion clinic.

(3) The appropriate license fee.

(c) Upon determination by the commissioner that the applicant has met the licensing requirements for an abortion clinic, the commissioner shall approve the application for the renewal of a license to operate an abortion clinic and issue a license.
