Sec. 5.

(a) If a health officer is notified in writing by a physician of a patient for whom the physician has medical verification that the patient is a carrier, and who, in the best judgment of the physician, is a serious and present danger to the health of others, the health officer shall make an investigation of the carrier to determine whether the environmental conditions surrounding the carrier or the conduct of the carrier requires the intervention by the health officer or designated health official to prevent the spread of disease to others. This investigation shall include the following:

(1) A determination of the environmental conditions or specific conduct of the carrier that pose a risk of spreading the disease.

(2) A determination of the epidemiological significance of the risk of spreading disease caused by the environmental conditions or the conduct of the carrier.

(b) If it is determined, following the investigation, that the condition or conduct warrants further intervention, this action shall be handled by the local health officer or referred to the department for further action.
