Sec. 15.

"Primary care case management" or "PCCM" means a delivery system for health care in which members are linked to a PMP who contracts directly with the office. The PMP is responsible for coordinating designated covered services, and he or she, as well as all other providers rendering services in this delivery system, is reimbursed on a fee-for-service basis.
