Section 4-2 - Certificate of registration

Sec. 2.

(a) An application for a certificate of registration as a waste tire transporter must:

(1) be completed and submitted on a form provided by the department; and

(2) contain the information in IC 13-20-14-5(a) and the following information:
(A) The business owner telephone numbers.

(B) The business owner email addresses.

(C) A list of all locations where tires will be taken that includes the following:
(i) Name of the location.

(ii) Street address.

(iii) Telephone number.

(iv) Type of location, such as re-tread facility, landfill, registered waste tire facility, or used tire dealer.

(D) A brief description of the waste tire transporting operations, explaining the anticipated sources, types, and quantities of waste tires transported.

(E) A copy of all out-of-state registrations, if applicable.

(F) A business owner certification statement.

(b) The fee required by section 3 of this rule [sic, section 3 of this rule] must be submitted with the application form when the form is submitted to the department.

(c) An application for a certificate of registration must be submitted as a paper copy unless the commissioner makes a determination that a copy in an electronic format as prescribed by the commissioner and in accordance with IC 13-14-13 is sufficient.
