Sec. 7.
(a) The commissioner may revoke the accreditation of a training course provider if the provider:
(1) violates or fails to meet a
requirement of this rule;
(2)
falsifies information on an application under section 2.1 of this rule to
provide training courses for operators;
(3) falsifies certified operator information;
or
(4) fails to provide a quality
course based on an audit by department personnel.
(b) A revocation may be appealed under IC 4-21.5.
