Sec. 14.
(a) Effective June 29, 1993, a public water system that uses a surface water source or a ground water source under the direct influence of surface water and provides filtration treatment must report monthly to the commissioner the information specified in this section. Systems shall submit information to the commissioner using the methods specified in section 13(e) of this rule.
(b) Turbidity measurements as required by section 8.8(b) of this rule must be reported within ten (10) days after the end of each month the system serves water to the public. Information that must be reported includes the following:

through (3) for at least twelve (12) months.
(d) Each system, upon discovering that a waterborne disease outbreak potentially attributable to that water system has occurred, must report that occurrence to the commissioner as soon as possible, but no later than by the end of the next business day. If at any time the turbidity exceeds five (5) NTU, the system must consult with the department of environmental management as soon as practical, but no later than twenty-four (24) hours after the exceedance is known in accordance with the public notification requirements under 327 IAC 8-2.1-9(b)(3). If at any time the residual falls below two-tenths (0.2) milligram per liter in the water entering the distribution system, the system must notify the commissioner as soon as possible, but no later than the end of the next business day. The system also must notify the commissioner by the end of the next business day whether or not the residual was restored to at least two-tenths (0.2) milligram per liter within four (4) hours.
