Sec. 17.
(a) A wastewater treatment plant operator certificate:
(1) is issued after an applicant successfully
completes the examination for the appropriate classification;
(2) must specify the:
(A) month and year that the applicant
qualified; and
(B) date the
certificate was issued;
(3) must be validated by a current
certification card; and
(4) is not
valid if obtained through:
(A)
fraud;
(B) deceit; or
(C) the submission of incomplete or
inaccurate data on the examination application.
(b) A certification card:
(1) may be issued for a time period of not
more than thirty-six (36) months; and
(2) expires on the date printed on the
card.
(c) To obtain a replacement or duplicate certificate, a certified operator shall submit a written request to the commissioner, including the following information:
(1) The class of the wastewater treatment
plant operator.
(2) The date of
issuance of the original certificate, if known.
(3) The certificate number.
