Sec. 15.
Neither short-term nor long-term disability benefits are payable for a disability resulting from the following:
(1) War, declared or undeclared.
(2) Participation in a rebellion, insurrection, or riot.
(3) Intentionally self-inflicted injury.
(4) Commission of, or attempt to commit, a crime.
(5) An accident caused by intoxication due to alcohol or illicit drugs.
(6) Willful failure to follow designated safety procedures.
(7) Willful disregard of rules.
(8) Willful failure to perform a stated duty.
