Sec. 11.
(a) Written policy, procedure, and practice provide that staff prepare a disciplinary report when there is a reasonable belief that a juvenile has committed a major rule violation or reportable minor violation.
(b) An appropriate investigation is begun within twenty-four (24) hours of the time the violation is reported and is completed without unreasonable delay. Exceptional circumstances for delaying the investigation are documented.
