Sec. 18.

(a) All significant incidents, including, but not limited to, the nonroutine use of restraint equipment and the use of physical force, are reported in writing prior to the end of the shift and signed with a dated signature by the staff person reporting the incident.

(b) A copy of the report is placed in the juvenile's case record and is reviewed by the facility administrator, or their designee, and the parent agency on the next business day.
