Section 240.760 - Social Security Number

a) To be eligible for CCP, each participant must furnish a Social Security Number (SSN). If more than 1 SSN has been used by a participant, then all SSNs are to be furnished.

b) If any CCP participant does not have an SSN, the Department or CCU shall assist him/her in making the application.

c) CCP services will not be denied, delayed or discontinued pending the issuance or validation of an SSN if the participant has applied for the SSN.

d) Participants who refuse to furnish an SSN, and/or apply for an SSN when requested, are ineligible for CCP.
