Section 604.315 - Source Water Assessment

a) The source water assessment must contain the following information:
1) a statement of the importance of the
source water;
2) a list of water
supplies that obtain water from this community water supply;
3) a delineation of all sources of water used
by the community water supply, including:
A)
for surface water, description of the watershed, map of the watershed, and
intake locations;
B) for
groundwater, the well identification number, well description, well status, and
well depth; a description of setback zones; and a description of the aquifer
for each well;
4) a
report on the quality of the source water for all sources of water delineated
in subsection (a)(3), including:
A) when and
where samples used to determine the quality of the source water were taken.
These samples must be tested by a certified laboratory; and
B) the certified laboratory's
results;
5) a report on
the quality of the finished water;
6) an identification of potential sources of
contamination to the source water;
7) the analysis of the source water's
susceptibility to contamination; and
8) an explanation of the community water
supply's efforts to protect its source water.
b) Upon request, the Agency will provide technical assistance to a community water supply in conducting the source water assessment.
c) A community water supply may use a Source Water Assessment Program Fact Sheet prepared by the Agency to fulfill the requirements of this Section.
