Section 340.1230 - Reports of Exposures, Radiation Levels and Concentrations of Radioactive Material Exceeding the Constraints or Limits

a) Reportable Events. In addition to the notification required by Section 340.1220 of this Part, each licensee or registrant shall submit a written report to the Agency within 30 days after learning of any of the following occurrences:
1) Incidents for which notification is
required by Section
340.1220
of this Part; or
2) Doses in excess
of any of the following:
A) The occupational
dose limits for adults in Section
340.210
of this Part; or
B) The
occupational dose limits for a minor in Section
340.270
of this Part; or
C) The limits for
an embryo/fetus of a declared pregnant woman in Section
340.280
of this Part; or
D) The limits for
an individual member of the public in Section
340.310
of this Part; or
E) Any applicable
limit in the license; or
F) The
ALARA constraints for air emissions established pursuant to Section
340.110(d)
of this Part; or
3)
Levels of radiation or concentrations of radioactive material in:
A) A restricted area in excess of any
applicable limit in the license; or
B) An unrestricted area in excess of ten
times any applicable limit set forth in this Part or ten times any applicable
limit set forth in the license, whether or not involving exposure of any
individual in excess of the limits in Section 340.310.
b) Contents of Reports
1) Each report required by subsection (a) of
this Section shall include a description of the event, including the date, time
and location of the event, the manufacturer and model number of any equipment
that failed or malfunctioned and the identity, quantities and chemical forms of
any radionuclides involved. Each report shall also describe the extent of
exposure of individuals to radiation and radioactive material, including, as
appropriate:
A) Estimates of each individual's
dose;
B) The levels of radiation
and concentrations of radioactive material involved;
C) The cause of the elevated exposures, dose
rates or concentrations; and
D)
Corrective steps taken or planned to ensure against a recurrence, including the
schedule for achieving conformance with applicable limits, generally applicable
environmental standards and associated license conditions.
2) Each report filed pursuant to subsection
(a) of this Section shall include for each individual exposed: the name, Social
Security account number and date of birth. With respect to the limit for the
embryo/fetus in Section
340.280
of this Part, the identifiers shall be those of the declared pregnant woman.
The report shall be prepared so that this information is stated in a separate
and detachable portion of the report.
