Subpart E - EQUAL OPPORTUNITY SCHOLARSHIP PROGRAM
Section 2772.520 - Application Process

Applications must be completed on forms provided by the State Board of Education and must be submitted by the applicable date specified on the form. Completed applications shall include the following information.

a) Name, address and Social Security Number of the applicant.

b) An indication of the applicant's membership in one of the groups designated in Section 2772.510(b) of this Part.

c) A certified transcript showing any postsecondary work completed.

d) A statement by the certification officer of the institution attended that the applicant is enrolled in an approved program of educational administration.

e) An indication of the term and the type of certificate of endorsement for which the scholarship is sought.

f) A statement by the institution's financial assistance officer of the total cost of the tuition and nonrevenue bond fees for the requested courses.

g) A statement signed by the applicant that the information supplied by the applicant is true.
