Rule 16.02.10 - IDAHO REPORTABLE DISEASES
Section 16.02.10.040 - REPORT CONTENTS AND METHOD OF REPORTING

01. Report Contents. Each report of a reportable disease or condition must include:

a. The identity and address of the attending licensed physician or the person reporting;

b. The diagnosed or suspected disease or condition;

c. The name, current address, telephone number, birth date, age, race, ethnicity, and sex of the individual with the disease or other identifier from whom the specimen was obtained;

d. The date of onset of the disease or the date the test results were received; and

e. In addition, laboratory directors must report the identity of the organism or other significant test result.

02. How To Report. A report of a case or suspected case may be made to the Department or Health District by telephone, mail, fax, or through electronic-disease reporting systems as listed in Section 030 of these rules.

03. After Hours Notification. An after hours report of a disease or condition may be made through the Idaho State EMS Communications Center (State Comm) at (800) 632-8000. A public health official will be contacted regarding the report.
