To preserve and advance universal service, protect the public safety and welfare, ensure the continued quality of telecommunications services, and safeguard the rights of customers, upon the petition of any telecommunications carrier, any customer, or any other person, or the commission's own motion, the commission may, after notice and hearing, suspend or revoke, in whole or in part, a COA or COR, if the certificate holder:
(1) Fails to make contributions required by the universal service fund;
(2) Fails to provide adequate telecommunications service;
(3) Fails to maintain access to emergency services; or
(4) Violates any applicable:
(A) Federal laws or regulations;
or
(B) State laws or commission
orders or rules.
