Section 17-2000-43 - Contents of petition

(a) The petition shall state the following:
(1) Name, address,
telephone number of the petitioner and the petitioner's legal counsel, if any,
which shall be updated by the petitioner at all times;
(2) A brief and concise factual statement of
the petitioner's claim;
(3) The
law or rule involved;
(4) The names
of all respondents or identities against whom the petition is brought;
and
(5) A brief statement of the
relief sought by the petitioner.
(b) If the petition is not in substantial compliance with subpart (a), the board may refuse to file the petition and may request the petitioner to submit an amended petition in compliance thereto.
