(a) Admission procedures shall require that sufficient information and instruction from the parents or guardians be furnished to enable the caregiver to make decisions or act on behalf of the child.
(b) Prior to admission of a child to a facility, the provider shall obtain in writing from the child's parents or guardians the following information:
(1) The child's full legal name, birth date,
current address, and preferred names;
(2) The child's likes, dislikes, allergies
and other personal information which may impact on the child's over-all
behavior;
(3) The name and address
of the parents or guardians who are legally responsible for the
child;
(4) Telephone numbers or
instructions as to how the parents or guardians may be reached during the hours
the child is in the child care facility;
(5) The name, address, and telephone number
of persons who shall assume responsibility for the child if for some reason the
parents or guardians cannot be reached immediately in an emergency;
(6) The name, address, and telephone number
of persons authorized to take the child from the facility.
(c) The information shall be available on facility forms and shall be updated as necessary.
