(a) Information pertaining to an individual child or parents or guardians of the child shall no be disclosed to persons other than the facility staff or the department, unless the parents or guardians of the child grant written permission for the disclosure or an emergency arises.
(b) The facility shall have release of information forms available for parents or guardians to sign.
(c) The parents or guardians shall be informed in writing of the facility's policy regarding disclosure of information.
