(a) A facility shall notify parents or guardians and the department of any changes in the child care services it provides. Notification of any changes in service shall be made no later than thirty days before the date of the change, and the changes shall be included in the facility's operating policies.

(b) The provider shall inform the department in writing of changes in staff members who provide direct care to children within five working days after the change.
