The department may provide up to twelve months of transitional support services to former FTW program participants, provided the participants do not refuse such services. Services shall include but are not limited to:

(1) Case management services designed to give the former FTW program participants a steady contact for assistance; and

(2) Transportation services for the former FTW program participants to enable them to retain their full-time employment. This may include bus passes, mileage reimbursement, and reimbursement of costs for other means of transportation as approved by the department.
