Section 17-2-4 - Commencement of proceedings in request for hearing

A proceeding shall commence by the filing of a request for hearing with the department either in person or by mail. The request shall be made in writing, dated and signed by the petitioner or an authorized representative, and shall contain a short and plain statement of the relief requested and the reasons therefore. When a request is filed, the department shall record its receipt and assign it a docket number.
