(a) Sleep services are services provided for the diagnosis and treatment of sleep disorders and shall:
(1) Be performed by sleep laboratories
or sleep disorder centers; and
(2)
Be provided to Medicaid eligible individuals only when ordered by a physician
and authorized by the department on form DHS 1144.
(b) Medicaid payments shall only be made for sleep services furnished by sleep laboratories or sleep centers who are accredited by the American Sleep Disorders Association by January 1, 1997.
