Section 17-1736-20 - Provider requirements prior to certification

(a) Prior to certification as a provider under the medical assistance program, a provider must submit to the department full and complete information about:
(1) The ownership of any subcontractor with
whom the provider has had business transactions totaling more than $25,000
during the twelve month period ending on the date that certification is
requested; and
(2) Any significant
business transactions between the provider and any supplier of services or
goods wholly owned, or between the provider and any subcontractor during the
five year period ending on the date of the request for certification.
(b) The information required by subsection (a) must be provided within thirty-five days of the date of any request by the secretary of DHHS or the medicaid agency of the State.
