The following information shall be entered in the record of each refugee:
(1) The name and address of the sponsor;
(2) The name of the national voluntary resettlement agency which resettled the refugee or the absence of an involved voluntary resettlement agency; and
(3) The passport or alien registration number on the form I-94.
