(a) Any institution whose certificate of approval is denied, suspended, revoked, or not renewed by the department may request a hearing, in writing, as specified in chapter 17-602.
(b) The written request shall be received by the department within ninety calendar days of the date the department mailed the letter of denial, suspension, revocation, or non-renewal to the institution.
