The child-caring institution shall immediately report to the department, any changes that may impact the institution's certificate of approval or its ability to carry out the requirements of this chapter. These changes include, but are not limited to, changes in program, services, organizational structure, location, personnel, or criminal history or background of the applicant or employees.
