(a) Persons whose application for a certificate of approval has been denied or a resource family whose certificate of approval has been revoked, suspended, or not renewed by the agency may request a hearing in writing, in accordance with departmental procedures.
(b) The written request shall be received by the agency within ninety calendar days of the date the agency mailed the letter of denial, revocation, suspension, or non renewal to the applicant or resource family.
