Each applicant for a private detective or guard license and each general partner, member of a joint venture, manager member of a limited liability partnership or company, or corporate officer listed on the agency application shall submit on a form provided by the board, a notarized statement covering the applicant's occupational and employment history for the ten years immediately preceding the date of application.
