Section 16-90-8 - Application for examination

An applicant for examination for a license as a nursing home administrator shall:
(1) Apply in writing on forms that are:
(A) Provided by
the board; and
(B)
Notarized;
(2) Apply at least two months prior to the examination;
(3) Provide evidence satisfactory to the board that the applicant meets the pre-examination requirements provided for in chapter 457B, HRS, and this chapter; and
(4) Submit with the application:
(A) Three letters, attesting to
the applicant's good moral character, from individuals engaged in either
business or the professions, who are not the applicant's relatives or
employees; and
(B) A finished,
unmounted photograph of the applicant which shall not be less than two and
one-half inches nor more than three inches square, and which shall have been
taken within three months prior to the date of the application.
