(a) An application, accompanied by the required fees and filed with the board, shall be prepared in accordance with and contain the information called for in the application form prescribed by the board, or any instructions which may be required by the board.
(b) The executive secretary or such other person authorized by the department shall determine the sufficiency of qualifications of applicants.
(c) The applicant shall attach to the required and appropriate application a recent passport sized photograph.
